Would this reinstall OS X, or thats something different?

It's different. You don't need to reinstall the OS.

Any recommendations for RAM updates?

I don't know that you need a RAM update. If the Memory Pressure graph in Activity Monitor always stays in the green zone, you have enough memory. I hope you don't believe the nonsense on this site that more than 4 GB is always required to run Yosemite adequately. Sometimes it is, but that depends on the usage pattern.

Actually the activity Monitor is showing that

Kernel_task is currently 486.3 MB (from 2.0 GB)

Wired Memory 628.5 MB (from 1.01 GB)

Those values are not significant. Your kernel usage will go up and down. Same with wired memory. Activity Monitor only report the status of your machine while you are looking at it. To see what is really going on, you have to run Activity Monitor while your machine is slow. If your machine is like other 4 GB machines running Yosemite, it may take some time just for Activity Monitor to launch in these situations.

Your machine has been cannibalizing itself due to insufficient memory. That is what it is supposed to do when it runs out of RAM. But if your machine is running slowly, and is clearing saying that it is running low on memory, then you need more memory.
