If you have a VPN set up and connected on your iPhone and then connect another device to the iPhone hotspot, I would imagine that all traffic is routed through the VPN. The iPhone handles the request from the device, sends out the request through the VPN, receives the response and then sends it to the device. The connection between your iPhone and other device is of course different and not routed through the VPN.

Edit: I now see that you want to share the connection via Wireless. Unfortunately the personal hotspot only works with your data provider. Why not just connect your other device to the Wi-Fi as well?

Just FYI, a VPN uses the internet.

It is not clear which direction you're sharing your network connection to...

Computer internet to iPhone - I think it is possible as long as the VPN is connected first, then internet sharing activated.

iPhone to computer - unknown - no point to attempt such as the computer can connect to the VPN using the hotspot internet connection.

I should note that the iPhone has native VPN Support - About VPN protocols for iPhone, iPad, and iPod touch - Apple Support

From Apple's "How to Write a Good Question":

To help other members answer your question, give as many details as you can.

  • Include your product name and specs such as processor speed, memory, and storage capacity. Please do not include your Serial Number, IMEI, MEID, or other personal information.
  • Provide the version numbers of your operating system and relevant applications, for example "iOS 6.0.3" or "iPhoto 9.1.2".
  • Describe the problem, and include any details about what seems to cause it.
  • List any troubleshooting steps you've already tried, or temporary fixes you've discovered.

You have us guessing what Apple product you have--not a good idea around here.

Also, your post found its way to a seldom-viewed forum for a long-obsolete Apple productivity software suite. When we know your Apple product we can ask the Hosts to move your post to a more active and appropriate forum where someone may actually see it.

Well, one reason might have to do with being overseas trying to tether some device like a Roku and needing it to route thru the VPN so a provider like, say, Hulu will see the US-situated IP of the VPN server rather than the overseas IP. The ROKU can connect to the iPhone hotspot, but the moment you enable the iPhone hotspot you disconnect the VPN (and vice versa). The Roku can connect to the hotel wifi readily enough, but has no VPN client capability

It's probably moot though. I don't think the iPhone's hotspot is likely to provide enough bandwidth for video streaming anyway.

If you're outside of the U.S. and trying to use Hulu, you're in violation of their Terms of Use:

Whether or not you've paid your bill is not the point.
