I have turned on the option to mirror my iPhone for Airplane Mode.
This works when you turn Airplane Mode ON, it is then turned on in both devices.
However, turning Airplane Mode OFF on the iPhone does not turn off Airplane mode for the Apple Watch.
Does anyone else have this issue?

When Bluetooth and Wi-Fi are disabled on your iPhone and your Apple Watch reports that it is connected, then your Apple Watch has connected, as a fallback, to a known Wi-Fi network:
I can see the logic in that answer. But I just carried out a bit of an experiment; I set airplane mode to on from the watch and both the watch and the phone went into that state. I then turned airplane mode off on the watch. As you would expect it appeared to not be able to send that state to the 'phone (i.e. the phone stayed in airplane mode). But.... the watch reports that it is connected!!! If ALL radio is off why does the watch think it is connected to the phone?
i think its either done that way on purpose - or an example of having to rush the software out without it being completely polished - mine works that way too
Official response from Apple Support:
For that Airplane mode, that is normal behavior, it can be activated through the phone, but will need to be disabled on each device manually.
I have the same problem, except that I want to be able to control the airplane mode on both devices from my Watch. I use airplane mode multiple times a day, so having to turn it off manually on both devices is a major pain. ðŸ˜
Put in a New Feature Request at apple.com/feedback. They have a dedicated team looking at these suggestions.
It's not a design flaw. By its very nature, Airplane Mode turns off all the radios on both devices. When you turn on Airplane Mode, it can tell the other device to also enter Airplane Mode, but once the radios are off, there is no way for them to communicate, hence having to turn Airplane Mode off on both devices.

My watch is locked, so I can't turn off the Airplane mode. It says "To unlock Apple Watch, first disable Airplane mode." Then, when I try anything else, it says that it is locked. Help - the phone is now useless!

I'm experiencing the same issue. I have no idea why Apple thinks that that's normal or appropriate behavior for those devices. If anything, it's counterintuitive. In fact, in earlier versions, I seem to recall when I turn airplane mode off in my watch, airplane mode was also disabled and my iPhone. Not the case now.
Try this to disable Watch's Airplane Mode...
Click on the Watch's top side button until you see the app icons, tap on the settings icon, tap Airplane Mode, tap slider to off.
That's it.
Above should read "system preferences" and not "settings."

That is certainly one of the methods via which Airplane Mode can be turned off on Apple Watch (via Settings).
However, the questions here (which have already been answered) were:
- Why doesn't turning Airplane Mode off on one device also turn it off on the other? (answer: that is the expected behaviour, as it is not possible).
