Forward and back buttons...not shown

The forward and back buttons, along with other toolbar option buttons are not shown. I can hover over the spaces and see the pop-up but the space is whited-out. It's not only in Safari I've experienced that but in other areas, like Activity Monitor for example. Has this happened to anyone else and can someone please assist?

Thanks

MacBook Pro (13-inch Mid 2012), OS X Yosemite (10.10.3)

Reset PRAM. http://support.apple.com/kb/PH18761

Reset SMC. http://support.apple.com/kb/HT3964

Choose the method for:

"Resetting SMC on portables with a battery you should not remove on your own".

Start up in Safe Mode. http://support.apple.com/en-us/HT1564

Safari

Click "View" menu in the Safari menu bar and choose "Customize Tool".

Drag the default set into the toolbar.

If this doesn't help,

Safari > Preferences > Extensions

turn all extensions off and test.
