How do I return my iphone 5 to default settings

I have upgraded my iphone and wish to delete all information from my old iphone 5 before I part with it. It wont allow me to do so as it wont recognise my icloud password despite the email address and password being correct.

I have been online and checked my settings, icloud recognises the log in on the pc ????
