Forgot my restrictions password and now unable to reset my device

Hi, i have recently purchased a macbook and am handing my iPad to my father, however when attempting to wipe and reset the device for him i cannot remember my restriction passcode, i remember the standard passcode as that is entered daily, have tried various 4 digit pins that i use, no joy......

any ideas??
