Based on the information provided, it's impossible to say.
I'm sorry for the confusion here. My question is in reference to an iPhone 5s. An app that was always on the home screen suddenly is gone. I find it nowhere in the settings, etc. What could have happened?

Have you checked other screens? Folders?
