How can I set Safari to always request desktop sites? If not possible, how can I bookmark the desktop site (not mobile site)? Thanks.

If you see the mobile website and want to see desktop version of website
Simply tap the address bar to bring up your bookmarks and recent history
then swipe down to reveal some hidden option at the top
find request desktop site and tap on it now safari will be reloaded the web page and you can see the desktop version of website
I could not find this on Apple's support site, the feature has changed the way it is implemented in iOS9.
When on a mobile site, touch and hold on the refresh symbol on the right side of the address bar. It will show "request desktop site"

The second question was about do you have to do this for every bookmark, I have not tried this solution more than once, please post back with your results.

There is no global setting for iOS Safari to force automatic request for full site every time you navigate to a new site. Most sites should remember your setting choice in cookies, so once you've requested the full site, it should take you back to that unless you delete cookies. But, not all sites behave that way, and yes, then you must request the full site overtime you go to the site.
