iOS 9.1 glitch - 'restrictions' and passcode mysteriously activated eliminating icons and access to apps

i recently noticed that I was missing app icons from my iPhone 5, starting with the App Store icon. (Siri: 'You do not have an app called App Store. Would you like me to find it in the App Store?'). After restarting phone I noticed that another icon had gone missing (and possibly more), but while trying to troubleshoot I noticed that 'restrictions', including a passcode, was turned on. I have never activated 'restrictions' nor set a passcode for this function. Advice online suggested the icons/apps were turned off in the 'restrictions' function. My attempts to guess the passcode were unfruitful. I finally gave up and contacted Apple support where I learned the only way to fix the problem was to do a complete reset of the phone.

KKilted, I was told by Apple support that it was a software glitch, which sounds really improbable to me. Could my phone have been hacked? (Sorry about your name misspell—I am using my iPad and if I backspace or reposition the cursor to correct errors, I am no longer able to enter text!). I live alone and no one has access to my devices so I would have never used the restrictions function, and setting restrictions and entering a passcode seem impossible to have happened randomly in my pocket or purse. This whole adventure has frustrated me beyond belief. I have spent several hours now resetting, recovering data (some of which I have lost forever) and trying to get all my apps set up again, etc.

It is highly improbable. If your phone was not jailbroken, then there is no chance that it was hacked. Someone had to physically set the restrictions. If you restored a backup made on another iOS device that had restrictions turned on, then they restriction settings would have been restored to your phone. The restriction settings are contained within the backup. It is extremely unlikely that it was set accidentally as doing so requires setting a restriction passcode and verifying that passcode in order to turn the restrictions on.
