How do I know if my Mac Pro is effected by the recall?

Is there some place to go to check if my Mac Pro is effected by the bad video card recall?

In the notice, Apple notes that Mac Pros manufactured between February 8, 2015 and April 11, 2015 are eligible for repairs due to issues with the machine’s graphics cards that“may cause distorted video, no video, system instability, freezing, restarts, shut downs, or may prevent system start up.”

You can enter your serial number in several sites such as this one operated by sellyourmac.com, and get an idea of whether yours was built during the affected time:

Model:	- Mac Pro (Early 2009) (CTO)
Codename:	- No codename assigned.
Build Country:	- We don't know where this unit was built.
Build Year:	- This unit was built in 2010.
Build Week	- Your Mac was built in week 16 of that year (April).
Production Nr.:	- This unit was number 49 to be built that week.
