how can I create a self-signed Code Signing Certificate on OS X Server on a Mac Mini without having a third party involved?

Mac Mini Server (Late 2012), OS X El Capitan (10.11.5)
Server.app will generate its own self-signed Code-Signing certificate automatically. However if you want more control over that I have successfully used the free XCA tool to create a self-signed one. This allowed me to specify a longer lifespan as an example.

While this is fine for signing Profile Manager profiles it is not a lot of help with regards to signing installer packages. That requires something a little different - an official Apple developer certificate.
