Why is my Macbook Pro fan too loud?

I have been using my MacBook Pro (13-inch, Mid 2012) for about 3-4 years now, for the first year it was fine no loud fan noise of anything but after a year it started making loud fan noise whenever I open more than a few apps. And it has finally came to my concern that i should do something about it since it heats up so often.

Is there anything I can do to avoid it?

Is the fan replaceable? If so would it fix it?

1. SMC - Dominic's right, start by resetting the SMC.

2. External reasons for overheating - Make sure your adapter is plugged into a wall outlet, not into an extension cord. Make sure the computer is on a flat surface. Make sure the computer is clean (you can use a soft brush or compressed air).

3. Software - Check the Activity Monitor (Use Activity Monitor on your Mac - Apple Support) to make sure there is not one of your apps which takes up all your processor and RAM for no reason. If it applies, troubleshoot or uninstall said app.

4. Hardware - Do a test to check if your computer has a hardware issue (Using Apple Hardware Test - Apple Support).
