The glass of my iPhone 6 is separating from the case. How do I go about rectifying this issue and what might be causing this to happen?
I take meticulous care of my phone. I always use a case and screen protector.

You should get the phone checked by visiting an Apple Store or an Apple Authorized Service Provider next to your place. Has your battery been replaced recently?
No I haven't had the battery replaced and it appears to work fine,
Get it examined by Apple, they will find the reason for that.
have had iPhones since 2g and only 5 and 6 have given me grief, with 5 it was the home button and with 6 it is the screen separating on right side. I am also very careful with my phone. issues always happen right when warranty runs out.
