As you can see in the screenshot below the pagination is aligned right sometimes in the table of contents, and sometimes it looks all wrong. What do I do to fix this?

Pages 5.6.2 on OS X El Capitan 10.11.5
Menu > View > Show Rulers.
You can select (click) each level (Style) of the Table of Contents and see the settings on the ruler.
With the second level selected, look in Format Panel > Text > Tabs

Might I also ask you about pagination in the rest of the document? I want to be able to display numbering in the footer from a certain page. Not sure yet whether to start the numbering from a later page than the first one, or to just start showing it from say page 3/number 3.

To start page numbering on Page 3, insert a Section Break at the end of Page 2.
Document Panel > Section > Page Numbering > Start at 3.
Delete the page numbers from the previous section..
