PUK locked?

I have an ipad that only has wifi (no 3g capability) I keep getting a message that says PUK locked. Insert a valid SIM with no PIN lock to activate iPad. How the heck do i do this as i have no idea what the code is and this ipad is not affiliated with any tele carrier?

Hi. If you have WiFi only iPad, it has no SIM tray, and iOS will never issue messages related to it. So this may be a hack. Does it appear on the home screen, or in Safari? If it's on the home screen, check for an unwanted profile in Settings > General > Profile. If there's no profile, you won't see this entry. If it's only in Safari, force quit Safari, then Settings > Safari > Clear History & Website Data.

It would show up in the home screen. I erased the contents of the ipad since I am was passing it onto another employee and the message comes up as soon as I start the ipad set up where you select your country or region. it will not let me progress beyond that point, and prompts me to enter my PUK. It says I have 10 attemps remaining but I have tried well over ten times, so maybe a virus is left on the ipad? Any idea how I can go nuclear and erase everything and start over?
