Did you clear History?
If you clear history, icons for webpages will be removed.
When you visit those sites again, those webpage icons will be restored.
Would you make a screen shot so people can see what you are seeing? Make sure you cover any personal information using Preview. Post the screen shot in a Reply using the camera icon or you can drag the screen shot into a reply. Copy and paste doesn't work.
Do you have this problem with other browsers installed on user system for example does chrome or firefox have this problem, or it only relates to safari?
