The following steps may help:
Check that Wrist Detection is turned on and that, if passcode is enabled, you are unlocking your watch after placing it on your wrist
- On your iPhone, in the Watch app, go to: My Watch (tab) > General > Wrist Detection - turn on.
On your iPhone, go to: Settings > Privacy > Motion & Fitness - check that both Fitness Tracking and Health are enabled.
On your iPhone, in the Watch app, go to: My Watch > Privacy > Motion & Fitness - check that Fitness Tracking and Heart Rate are enabled.
Check that you are wearing your watch sufficiently snugly on the top of your wrist:
- If it is worn too loosely, the accuracy of heart rate readings may be affected and your watch may believe that it has been removed from your wrist.
If you have not yet done so, update your iPhone and Apple Watch to the latest software, starting with your iPhone:
- Update the iOS software on your iPhone, iPad, or iPod touch - Apple Support
- Update the software on your Apple Watch - Apple Support
Restart both your iPhone and your watch:
- Close the Watch app, Health app and Activity app (if open) on your iPhone:
- Double-click the Home button, then swipe up on each app preview to close it.
- Turn both devices off together, then restart your iPhone first:
Unpair and re-pair your watch:
- The Watch app on your iPhone takes a backup of your watch automatically when unpairing via the app. Choose to Restore from Backup during set up. Most data and settings will be restored, with some exceptions (eg Apple Pay cards, passcode).

If the issue then persists, I suggest that you contact Apple Support (mail-in service may be available), make a Genius Bar reservation or visit an Apple Authorised Service Provider with a view to having your watch checked and, if recommended, serviced / replaced (whether under warranty, if eligible, or via chargeable repair service):
