Team ID is easy: from https://www.mobiloud.com/help/knowledge-base/ios-app-transfer/
- Log in to Apple’s Developer Center
- Once you are logged in, click on Account
- Then Membership
- Under Membership Information, please you'll find the Team Name and Team ID fields
The Team Agent Apple ID seems to be the email you've used to created the developer account .
