Team ID is easy: from https://www.mobiloud.com/help/knowledge-base/ios-app-transfer/


  1. Log in to Apple’s Developer Center
  2. Once you are logged in, click on Account
  3. Then Membership
  4. Under Membership Information, please you'll find the Team Name and Team ID fields

The Team Agent Apple ID seems to be the email you've used to created the developer account .
