“Your bootable USB Drive could not be created” Boot Camp error

Every time I use Bootcamp to install WIndows 10, I get this error:

“Your bootable USB Drive could not be created”


Steps I have tried to fix the problem:

1) Downloading the ISO directly from Microsoft

2) Downloading the older Anniversary ISO (as recommended by Microsoft)

3) Converting the ISO to DMG and then to ISO (as recommended here)

4) Using Mac recovery mode to do a permissions scan (as recommended here)

5) Using a different USB drive.


System:

OS: 10.12.5 (16F73)

MacBook Pro (Retina, 15-inch, Mid 2015) - MacBookPro11,5

MacBook Pro (Retina, 15-inch, Mid 2015), macOS Sierra (10.12.1)
