Apple Watch - band size

Does the band LENGTH that is found in the Apple Watch_Sizing_Guide.pdf document include the length of the Apple case (watch/face) or does that length need to be added to the band length?

For example, if I select the 38mm case AND the Milanese Loop band, does the combined case and band go up to

180mm (from chart in doc)

OR

218mm (= band length in chart plus case length of 38mm)?
