I am using iTunes 12.7.1.14
I have a Smart Playlist with the following settings.
The list has 936 songs. I occationly find new songs I like but when I 'love' them they are no longer added to this playlist. I can't find a rhyme or reason. Any thoughts?

It’s a bug in the current version of iTunes. Opening the edit window and clicking the OK button will force an update, but it won’t happen automatically, as it should.
Let Apple know and hope for a quick fix.

I've tried opening the Edit window and selecting OK but it never udpates. Not sure when the problem started. I submitted feedback. Hopefully it will be addressed.
