Charging Block Caught Fire!

This morning my daughters Apple charging block caught on fire. Luckily she was awake, we were home and no one was hurt. I would like for you to take a look at the attached photo, and let me know what your plan of action is to make sure this does not happen again. I have read and done some research and apparently this is not the first incident. This is very concerning to me and I am certain other customers would agree.
