Locked out of phone

My daughter is using an old iphone as an iPod and she forgot her passcode and we are completely locked out. We have done the hard reset and it still says it is disabled and to connect to iTunes. I have tried connecting to iTunes and that does not work either, it says it will not recognize the phone because it is locked out. Does anyone have any ideas?

Then you did it wrong 50 times. If you do it right it will work. See the section

Erase your device with recovery mode to remove your passcode and follow the instructions exactly to the letter.
