weird icon appeaed on iphone?

I've seen that before. One time was a hard link to a website URL (from Add to Home Screen) that I had saved. i don't recall exactly what it was (maybe the link was broken), but after a while it showed exactly that. Mine did have the original icon name though, but the icon looked just like that.

Have you tried selecting it?

The icon in the upper left indicates an app is in the process of being downloaded/installed on your iPhone. The process is under way. Once the app completes installation, you will see what it is.
