Are you using W10 1709? If yes, I suggest you use W10 1607. See http://windowsiso.net/ .
Disconnect all external storage, run SMC and NVRAM reset. Use the Lexar USB, and BCA-created USB Installer using W10 1607. Post any error messages you see.

Here are a couple of things i have tried.
Iso's
Win10 1607
Win10 1703
Win10 1709
Server2012eval
(All isos were downloaded using a wired ethernet connection)
Usb drives
Memorex 16GB 2.0
Lexar 8GB 2.0
Kingston datatraveler 3.0
Deleted all drive partitions and reinstalled OSX mavericks
Update to High Sierra
Created partition using BCA
Created partition using just Disk Utility
Created USB installer using BCA
Created USB installer using Rufus on Windows
Reset NVRAM
Reset SMC
Installed using windows 10 support assistant drivers
Also tried Windows 10 usb with windows 7 boot camp drivers
Deleted all partitions on SSD and tried installing only Win10 on the computer
I can get through the windows installer wizard and the computer reboots it and gets stuck at at getting ready
Leaving the USB installer connected or removing after final install reboot results in the same thing
1. Reinstalled OSX
2. Upgraded to High Sierra
3. Downloaded W10 1607 from (http://windowsiso.net/windows-10-iso/windows-10-anniversary-update-1607-download -build-14393-0/windows-10-anniversary-update-1607-iso-download-standard/)
4. SMC reset
5. NVRAM Reset
6. Used BCA and created W10 install disk (on Lexar 2.0)
7. Formated and clicked new on bootcamp partition
8. Started windows install
9. Started install
No devices plugged in other than install usb and power cord. No hubs or adapters.

Will update you once download is complete. Are the partitions created during the windows install step in the 3rd screenshot correct?
How long has it been sitting on this screen? If 1607 does not work, can you try release 1511?
