Accidental screen damage is not covered by the regular warranty. If you have Applecare Plus you get two accidental damages/incidents for the life of the warranty at reduced cost.

Cost will also depend upon service availability and your country. If you have to take it to a third party Apple Authorized place I believe they can set their own price.
