These prices apply only to repairs made by Apple. Other service providers may set their own prices.
Depends on any number of factors. If it's just the screen replacement, the price is listed in that link provided for going directly through Apple (Apple Store or mailed to a service center). If there's more damage they might not replace it without your agreeing to repair other stuff.
If you choose to go through an Apple Authorized Service Center, the service prices are set by the shop.

Those are the terms but if you want to keep being an AASP you would be well advised to stay to that price schedule. AASPs are also following the terms of the battery replacement and charging $29.
