How do I start account recovery?

I was told a long time ago there is a way to recover an account when you lost a trusted device and I know I was even in the process of filling out the form at some point online but ended up having to stop because I didn't have my credit card on me at the time. I need to recover this account because it has all my iTunes purchases on it so if someone could link me to that form or something that'd be great.
