Question: How to removed white frame (iPhone touch screen)

A white frame appears around each button I touch on the phone, as well as a voice over. My iPhone is locked and typing in my passcode doesn’t unlock the phone (Siri only says out loud the numbers I enter). Perhaps I accidentally turned on a special needs assistance? How do I log on/ turn off this special assistance feature?
