Are you using iCloud Photo Library on the iPhone? What are your settings in
Settings > Your Name > iCloud > Photos & Camera?
You are seeing the exclamation point, if Photos cannot download the original of a photo from iCloud Photo Library. If you have been running out of cloud storage, or if you do not have enough free storage on your iPhone, or if you do not have an internet connection this can happen.
A common reason for this problem is, if you have more photos in iCloud tan the iCloud storage permits. Then you need to delete items from iCloud, to make iCloud working again.
Another problem is "Optimize Storage". If you take more photos than will fit onto your device, Photos will remove high resolution originals from the iPhone and keep the originals only in iCloud. But if you keep taking photos, even the optimized versions will no longer fit onto the iPhone. Then you need to download the photos from iCloud to a computer and delete enough photos from your iPhone and from iCloud that the iPhone can download again the photos to the iPhone. You need at least one teeth of the size of the original photos stored locally on the iPhone. If your photos in iCloud need 100GB, your need 10GB on the iPhone to store the thumbnails.
