help! My iPhone 6S plus is hacked? Or is it jail broken

There is nothing of any concern here from what I can see and from my many years of experience. Can you tell me why you think that you have been "hacked?" It is pretty much near impossible to hack an iPhone unless you authorize someone to get into your device, in which case it's not a hack. If you're really that concerned, just erase the phone and set up as new or take it to the Apple Store. They will likely tell you the same thing, however.

Go to settings down to reset Restore iPhone back to factory settings

Then set back up again over your wifi.
