Strange icons on home screen

I have a slew of unusual icons on my home screen. When I try to delete, the "X" in the corner of the icon does not appear. When I click any of the icons, most result in either a white or black blank screen. Some have simple menus when clicked. The icons are white with a crosshair test-like pattern.

Has anyone seen this behavior? ...and how do I fix?

Also, I don't know if it is related or not, but when I open Safari, the screen looks zoomed in.
