I have several custom ringtones I added to my iPhone a couple years back. The latest iTunes 12.7.4.80 will not display them in the device ringtones list. Also if I drop a ringtone onto the list it appears, but doesn’t upload to the iPhone.
Okay. I solved it! I remembered that I have this iPhone media syncing app MacX MediaTrans. I used the app to delete the ringtones, add them back, and then delete them again. It wasn’t enough to just delete them. Then I went into iTunes and added ringtones successfully.

No. That’s just the basic use instructions. It doesn’t address this malfunction.
Have you tried the procedure in the "Add ringtone to device, iTunes 12.7 or later" section of the last link FoxFifth cited above? If so, what is the malfunction?
