I am using a mid-2012 MacBook Air with macOS High Sierra 10.13.6. I need all audio output to be routed to the right earphone as I am deaf in my left ear, but the "Play Stereo Audio as Mono" option does not do anything. I know it's not the headphones, since mono audio works perfectly fine with my phone.

Agreed. I have the same issue on a mid-2012 Macbook Pro 13" with macOS Mojave 10.14 Beta (18A365a). It's really quite unchanged when the check box is selected. I have full hearing, but am setting up for my monaural wife.
Listening to "A Day in the Life" by the Beatles. It's totally stuck in stereo even after starting over the song, etc.
Please take this report seriously as the affected may have difficulty identifying the issue.
