¿Can you restart in Safe Mode (hold shift at startup)?

Safe mode does a five-minute Disk Directory repair, then attempts to load MacOS. It requires your Userid and password to proceed. Screen updates will be wonky and slow, but ultimately accurate. By design, it loads the smallest possible set of Apple-only extensions, and that does not include graphics acceleration extensions.

Works in Safe Mode but not in regular mode, says, "It's something you added".

You say the Mac is 8 years old. If it's a MacPro that would be the 2010/2012 model 5,1. Is that correct? There are of course a number of possible reasons why it cannot boot. However, I believe more information is required in order to offer any helpful suggestions. Do you hear the startup chime and/or disk activity after you power on? If so, how long does the disk activity continue? Have you checked the USB ports to see if they are active? I'm guessing they are not. If there seems to be no activity at all it can be a sign of a dead GPU (Graphics Card). I experienced this recently, and installing a good GPU brought my MacPro 5,1 back to life immediately. There are several tests that can be done, as well as alternate start up possibilities, but I'll let the more senior members "chime" in on those options.

Hmmm... That sounds like it could also be a HDD issue, meaning that your Hard Drive might be on its way out. Do you have another drive with the same or different OS installed, or perhaps a Clone of your Work drive. If so then Press the "Alt/Option " key while starting. Release the key after you hear the chime. The computer will then present the StartUp Manager and you can select the disk you want to boot on. Obviously the GPU is OK, since you do seem to be viewing the Apple logo and the startup progress bar. Have you perhaps installed any new(er) software recently? If so please advise. Do you have any apps that start up with the boot? I'm obviously grasping at straws here, but I have no further suggestions at this time.

OSX Base system is the name given to an Installer image. It is likely a view of Internet Recovery, not a physical Drive.

That is a debugger call in the Human-Interface-Device daemon. It is most likely caused by your clicking. (Otherwise it may be a USB device problem.)
