I have been getting this and similar pop ups for several days. I have cleared my history numerous times and changed both the password to login to my Mac and the one for iCloud.
Nothing is working.
Some solutions reference a ‘First Aid’ option under ‘Preferences’ on Keychain, but I do not have that option.
Also, I make a habit of keeping the keychain locked but when I login to my computer, it is unlocked.
How can I solve this?

You probably need to create a new keychain.

The keychain automatically unlocks when you log in to your Mac. This shouldn't be a security issue, as you still need to enter your keychain password to authenticate in most cases.

If keychain is prompting to enter password multiple times , & pop - up is not going away when safari is opened .
Click on Go > Go to folder & type ~/Library then find a folder > keychains > remove first folder ( alfa numeric folder ) to trash . Restart & empty the trash , open system preferences go to internet accounts & once again configure web mail account , I cloud mail account by entering their passwords .This is applicable in Mac OS X El - Capitan and previous versions .
Open air port utility via spotlight > click on edit and enter your base station password .

And then restart the Mac by taking cursor on apple logo and click on restart , the keychain folder will once again created in the user library after some time .
To change the keychain password, go to System Preferences/Users & Groups and change your user account password.
I changed the User password yesterday, still had no access to change the keychain password via the edit menu. Called Apple support, they suggested the same things. Ended up getting disconnected from chat and lost all of my passwords. Thanks anyway.

I've tried that solution. Even on chat with Apple Support. In the edit menu, the option to change the keychain password is greyed out.

Thank you. This sounds very helpful. Unfortunately during my phone conversation with Apple, my entire keychain was deleted.
