In regards to your "Create and manage ringtones in iTunes", I experienced a slight difference when using iTunes 12.9 and attempting to add .m4r files to device. Copying files from finder, I was unable to directly paste into "Tones" folder on device, nor was I able to add these files to my iTunes library utilizing File >> Add to Library. In order to move the files onto device, iPhone XS running iOS 12.0.1, I had to paste to "Music" folder on device, which caused an automatic move of files to the "Tones" folder. Once completed, my .m4r files were immediately available. Hope this brings some clarification to potential confusions others may experience!

It *only works if you have "manually manage music and videos selected" under the "Summary settings" of the device.
Given this, FlyinGIJoe's tutorial is incomplete.
Also, if you haven't already set the above Summary setting, all tones previously on the device will be wiped.
I just spent 40 minutes with tech support. He thinks the new iTunes no longer allows you to do this. We tried using an older music library from time machine - no help. Tried to copy and paste into the tones folder - nothing.
He thinks you may be able to move music into Garage band and create the tones there. The older directions found online no longer work where you change the music format and pick the music duration.

Previously created ringtones (.m4r) are still on my iPhone and can (still) be used as ringtones; but there is no way to create or transfer new custom ringtones to the device.
Do you see anything listed in the Tones area of the device when connect to iTunes? Have you tried inspecting the file system of the device with the method shown in Removing hidden tones?

In that case removing them all, and then adding back along with your new one ought to work.

Work carefully through the relevant section ofCreate and manage ringtones in iTunes for the task you are trying to perform. Where exactly do you get stuck?

Well either things stay as they are, or you bite the bullet and try to fix them. Do you have copies of those tones stored on your computer? Are they ones you made yourself or purchased from the iTunes Store?

I have followed every step and still am unable to get the ringtone file from my mac to my iph
Tones on my divice only has the Tones that I already have, and I don’t want to delete them
I found copies of the old one’s, have made them myself
I've just made and added a new custom ringtone to my iPhone 6+ running iOS 12.1 with iTunes 12.9 on Mojave using the steps I've given in Create and manage ringtones in iTunes.

The "Tone" option does not appear in my iTunes!
