Having the exact same experience here. Without question, this issue began when I “upgraded” to iOS12. This issue has been devastating and needs to be resolved ASAP. I depend upon iMessage and can no longer trust/depend on the tool to send messages to the proper person. In the same way, I cannot be sure from whom texts are actually being sent.
There are two steps to fixing it:
- Stop sharing an Apple ID
- Update to iOS 12.1
Nothing other then last name.
No, my parents don’t have a home phone, and I don’t have their address info in their contacts. I don’t have their last names in my phone, they’re just mom and dad.
I've seen that cause trouble in the past but, it sounds as if it's definitely not your issue.

Accept that even though I’ve separated the accounts, it’s combining them again.
My particular issue is that I can only text the last person who texted me from the combined thread. The outbound msg only goes to that person even iif I choose the other from the separated contact.
Yes, created separate Apple ids for each family member, yet somehow iOS 12 is associating them and merging them.
If they are using the same Apple ID this will happen.
And if they were using the same Apple ID, it will happen.

You have been told multiple times. Don't do that. The root cause of your problem is sharing an ID.
But they will lose photos, notes and everything, unless it's able to be restored from a backup. Like, you can't just transfer it over, if you get what I'm saying, if they start completely blank.
I get what you're saying. That doesn't change the fact that as long as they are sharing an ID, you will continue to have problems.

Accept giving them separate accounts after they’ve had the same id, doesn’t fix it. So there isn’t a fix for this, until Apple comes out with one.
