My Apple watch is shown as a data source in the Activity app and the Heath app but my iPhone only appears in the Health app...How can I add my phone as a source in the Activity app?

Thanks but I'm looking to add phone data to the iPhone Activity app, not the watch app...at the moment, I get credit in the iPhone Activity app for exercise performed when wearing my watch but not when I'm just carrying my phone - my watch shows up as a source but not my phone.
You cannot. Nor would you want to. The Activity App, on your watch is just for your watch's data generated data. All its data is then added to the health App on the iPhone automatically.
The Health App on the iPhone is the central collector for all activity data.

Again. the Activity App only tracks watch's activity.
The Activity App on your iPhone is just a mirror of the watch's Activity App.
You cannot add the iPhone as a source.
For your iPhone's activity tracking you use the Health App.
