apple Id verification code problem.

I keep getting a message to verify my apple ID on my mac and when I try to verify I get the verification code on my phone but no place to type it in on the mac. I want to be able to forward my messages and facetime to my mac. It was set up just fine till a couple of hours ago it quit working.

If you see a password box, enter the password (ABC) followed by the code (123) or ABC123.
