Mojave app icons

Since updating to 10.14 Mojave I have this really annoying bug where my App icons disappear and also the 'show icon preview' inside View options does not work either, it just displays the File icon such as jpg or png.

iMac, Mac OS X (10.7.1), 27" i5

some software icons are still showing on the doc bar after closing them We already know that pined items on the doc bar will stay there even after you close the application and on the right side you can see the open application but after I upgrade to the Mojave I can see some application in the middle. They do not close even after you close the application.

URL icons have disappeared I like to save some UrLs on the desktop. I can then see them is various finder windows. The URLs now just show a blank page icon. All my other icons seem to be ok . They are visible on the screen. How can I get them back? They have been there for years. Using Mojave 10.14.6. Happened after the latest update. Can't move to new system because I need to run the old quicken for a while.

Safari 16.3 not showing icons from extensions and favicons macOS Big Sur Hi, yesterday I got an update for both, Safari and macOS Big Sur, I applied the updates in my iMac late 2014 and when I opened Safari noticed that the icons from the favorite section where gone... I did not took it too serious because it can happen, and normally it should be a thing of re open the website and the icon would load again, well not this time... Please note that only the icons are missing, the extensions works as intended, so it is a different reason that when both the icon and extension are gone. Anyway, I had no luck with both issues. What I've tried: Cleaning up Safari's cache from dev options. Deleting the Favicons cache files from: "/Users/Username/Library/Safari/Favicon Cache". Rebooting. Checking out if the issues happened with Guest account (it did). The issues persists in Safe Mode as well.... Is anyone experiencing this issues?
