How do I unblock individual websites

My general website setting for my phone is that all websites are blocked. Up until the update a few weeks ago, when I got the "website blocked" message, it gave me a number pad so that I could enter in my restricted password and unblock that website. Now, I get the message but with not way to unblock it.

1) Is there a way to set it up so that I am able to enter my password to unblock individual websites?

2) I also noticed that after I unblock a website, it stays unblocked permanently. How can I re-block a website for the future?
