Uninstall CleanMyMac3 and CleanMyMac4. Having a single copy installed on a Mac is bad but having two is really just asking for trouble.

Have you tried going to Safari/Preferences/Extensions and disabling the extension? Quit/reopen Safari and test.

See if you can find an update for the Last Pass extension.

You don't have to uninstall them. If you go to Safari/Preferences/Extensions, you can disable them, but leave them installed.

Yes. Clearing cache might have remove something that is required for that web site. Why did you clear cache in the first place.
