Hey baxjax

You can turn the setting off so that your ringer stays loud even if you lift up the phone.

Go to Settings

Scroll to Face ID & Passcode

Type in your Passcode

Scroll to Attention Aware Features and deselect it

Your volume will remain loud now even if you pick up the phone.

If you pick up the phone and are looking at it while it is ringing, that is by design. The phone is designed so that when the phone "recognizes" that you are looking at it, then the ringing sounds, as well as other notifications will decrease in volume.
