Safari Top Hit is a website I’ve never visited?

Hello. I noticed that my top hit is almost always a website I’ve never visited. Granted, they are “safe” websites like DiscoveryKids, HowStuffWorks and National Geographic. The problem is, I’ve never visited those websites before! But they showed up on my top hit anyway. I have the ‘Preload Top Hit’ setting disabled btw. Can somebody please tell me if this is how Apple itself sets it or is someone using my safari app remotely/am I hacked??

ALSO my bookmarks and history are still present even AFTER i’ve deleted my website data. While these are indeed websites I’ve visited BUT deleted from history, I can’t help but wonder if someone is hacking my phone and pulling out information from my Safari app. Can someone please tell me what’s going on?

Also, below is a picture of my safari search. Keep in mind this is after I’ve deleted all website data. It shows that HowStuffWorks is in my history but I’ve never even opened that website!!

Am I being hacked? I would appreciate an answer on this. Thank you.
