Noise cancelation being turned off did not fix my issue either. It does it no matter what I do and seems to work okay if I make sure my mouth is right in front of the bottom of the phone but has to be perfect. Slightly to the left or right causes the issue. It’s so screwed up and even speaking to tech support they act like they don’t know this is a hardware problem and like many many people have not complained. They wanted to bring in for repair but after research what they do and replacement phones have the same issue. So I paid an arm and a leg for a otherwise great phone but I think apple got too focused on features and forgot the purpose of a phone is to talk to people and when your phone cannot do this important thing well it’s a big problem. Wish apple would recall them or make it right for those who have had issues with this situation. Probably going to end up getting the iPhone 11 and hoping that isn’t something that is happening with the 11 as well.

I have a similar problem on my new iPhone XR. While talking on the phone with the phone to my ear and the mic in front of my mouth, people can hear me. If I so much as pivot the lower end of the phone down from my mouth, people can no longer hear me. It works fine when on speaker or using ear buds. Something definitely wrong with the mic fading off if not directly in front of your mouth.
I had my phone replaced. Same issue still continues.
Sadly I am wondering too about why I have stuck with Apple especially when the primary purpose of the device (a telephone), doesn't work particularly well.
Apple will never admit any wrong doings... or mistakes. So... Caveat emptor.
I 100% agree, I’m on my 3rd phone, still with same issue and no resolve. I have been on this case for 7-8 months. They tried to record a few profiles on the phone and send them to Apple two times, months apart, but the files are too big to send and never go thru. Such a runaround... they could have bought me 5 new IPhones with the labor of Apple employees and my time on this!
try putting LTE to data only....it fixed my problem.

I sent mine back for warranty repair/service. It was too annoying.
My Hands-free microphone is very quiet when I am talking to people they can barely hear me on my iPhone XR
I think Apple did all their testing using AirPods and not the actual phone. It’s unfortunate though as everyone I know had the same problem and only got sorted when they put on the tape at the back.
Tape on back? I did not see that.
The problem happened with me without a case on, and not covering the back microphone whatsoever. It is something else.

Try talking on the phone with the cover closed. As it uses the rear microphone when talking on the phone.
