I had the same problem with Outlook 2003. You must go to the Help Tab, click on About. Look to the bottom of the screen and locate for the Disabled Items Tab.

You will find the Itunes Add-Ins which has been automatically disbled resulting in your checked items not remaining.

Uncheck these, go back to the Advanced Tab and try checking again. they should remain checked. It worked perfectly for me.

If each time you try to synchronize your Outlook 2003 Contacts and Calendar to your iPhone nothing happens try the following steps. I've found these steps are the fastest solution until Apple releases a fix; hopefully with the next edition of iTunes.

A. Open iTunes

B. Open Outlook 2003

C. In Outlook 2003...
1. Enter Tools -> Options
2. Select Other
3. Select Advanced Options
4. Select COM Add-Ins
5. Select Outlook iTunes Sync Addin
6. Click OK
7. Click COM Add-Ins again and verify "Outlook iTunes Sync Addin" is still selected; if it’s not selected perform these optional steps, however if it is selected skip these optional steps and move on to step 8.

Optional Steps – Only necessary to perform this if the "Outlook iTunes Sync Addin" does not remain selected after following the previous steps
Still in Outlook 2003…
a. Enter Help -> About Microsoft Office Outlook
b. Disabled Items
c. Enable the Outlook iTunes Sync Addin
d. Click Close
e. Click OK
f. Start at the beginning of these steps

8. Click OK
9. Click OK
10. Click OK
11. Leave Outlook 2003 open

D. In iTunes…
1. Synchronize the iPhone
2. Everything selected should be synchronized. That should include Calendar and Contacts if those are selected.
3. You can then close or leave iTunes open; doesn't matter
4. Move on to the next set of steps.

E. In Outlook 2003...
1. Enter Tools -> Options
2. Select Other
3. Select Advanced Options
4. Select COM Add-Ins
5. Un-Select Outlook iTunes Sync Addin
6. Click OK
7. Click OK
8. Click OK

OK.....I've spent more than few hours working on this issue and here is what I've found and believe to true!

The problem seems to begin when you first activate the phone....if you had more that one mail profile on your PC. Like one for work and one for home.

Even removing and re-installing the all of iTunes (Apple Update, Apple Mobile, and iTunes) will not fix the issue.

Some of the key files for syncing are stored in c:\document and setting\"user name"\application data\apple\mobile support\cleintdata

In order to get the sync with Outlook working.....here is what I did:

1. Removed iTunes, etc.
2. Deleted the files in the ClientData folder mentioned above
3. Reset the phone deleting all files (You will lose all your settings including bluewooth, wireless, etc)
4. Re-installed iTunes 7.3.1
5. Made sure I only had one mail profile on the PC...if you are using the OST (offline option) make sure it syncs 100%- no errors.....if errors see my prior posting on repairing.
6. Reboot
7. Plug in the phone
8. Open iTunes
9. Decline original offer to sync
10. Manually select only contact and calendar sync.
11. Sync and enjoy.......

Best of luck......It seems to fix the issue caused my having multiple mail profiles. The iPhone remembers that original data.....thus fixing the PC files doesn't correct the problem. But fixing the PC and resetting and erasing the config. files on the iPhone works........GreytMacman
