I have been plagued by this issue for two months. In addition to trying everything mentioned on this thread, I also tried every solution listed at http://www.macmaps.com/WIFI1048.html and couldn't fix it.
Four days ago, I was trying to hook up a USB drive to my Cisco E3000 router and discovered that I couldn't access the web interface on the E3000. After two hours of fiddling, I discovered that when my husband hooked up a new DirecTv receiver, he failed to change its IP address. In a nutshell, we had two devices that were still configured with a 192.168.1.1 address. The weird thing is that everything on the network was working fine -- including other wireless laptops and my MacBook *if* it was hard-wired to the E3000.
I changed the IP address on the E3000 and my macbook's wireless hasn't dropped once -- even though I've flipped between wireless locations (work, hotel and home) at least five times over the last three days.

Yup, that did it! I didn't remove the batter, but I did remove the power cord and held Power button down for 5 seconds. For reboot, I held down the Command, Option, and "P" keys while holding the power button until the chime. From that point everything started normally. Thanks!
I have a 2009 MacBook and this is the only thing that worked for me (after a night of un-plugging the battery and zapping the PRAM).

Resetting the SMC worked for me. Instructions here: Intel-based Macs: Resetting the System Management Controller (SMC)
Way easier than pulling the battery from newer Apple laptops.
