Well, my problem was resolved. I must say everyone I dealt with went out of their way to be superhelpful. I get the feeling that this is a problem that AT&T could solve by internally making a change to their database. My ultimate solution was to go to the Knox/Henderson Apple Store where they pulled a replacement phone. (Interestingly, not just another 3gs off the shelf, but one in a smaller, plain box. They said it was new, as there are no refurbished 3gs phones yet, it just came from different inventory. After completing paperwork they activated the new phone for me.)
As a side note, I must say how pleased I am with Apple's customer service. I own a small business and made the decision to buy Apple products after using Dells for years. Between the business and family, I have a Mac Pro, 3 Macbook Pros, 2 Macbooks, 2 iMacs, 4 iPhones, and a number of iPods. This incredible customer service experience - actual smart people who try and do fix a problem - cannot be matched by Dell, Microsoft, or anyone else. I will continue to use Apple products almost exclusively.
