Spotlight not finding apps

For quite some time now Spotlight does not list apps, even when the full name of the app is typed (e.g. "safari"). I have tried to rebuild Spotlight multiple times without success, including moving the HD in and out of the folder list under "Privacy", using Onyx and even using terminal to rebuild. Any suggestions on what could be wrong and how to fix?

iMac 17,1

MacOS Mojave 10.14.2

iMac Line (2012 and Later)

You do have Applications selected in the System/Spotlight preference pane, right?
